【摘要】:ThisTechnicalReportisconcernedwithQoSasitappliestointeractionsbetweencomponentsofdistributedhealthcareITsystems.Thescopeisnotlimitedtonetworkinfrastructures;itincludestheQoSrequirementsofinformationstorageandprocessingITsystems.Therelatedareasofsecurityandfinancialcostconsiderationsarenotwithintheprimaryscopeofthedocument,althoughtheyareconsideredbriefly.Ofcourse,aninformaticssystemwithahighQoSdoesnotguaranteeahighstandardofhealthcareintermsofclinicaloutcomesorpatientcare.Thequalityofhealthcaredeliveredtopatients(theultimate”users”)dependsuponanumberofexternalfactorssuchastheexperienceandcompetenceofthehealthcareprofessional(s)orinstitution(s)involved.PotentialQoScharacteristicsforthetotalhealthcaredeliveryprocesssuchasmortalityrate,clinicaloutcome,etc.arethereforenotwithinthescopeofthisreport.ThereportcontainsnoprovisionstoavoidtheincorporationofbadordangerouspracticeintohealthcareITsystems.Itispossibletocircumventgoodclinicalpracticewithtechnicalsolutionswhichmaycausebadpractice.Thisvitalissueisnotcoveredbythisreport.Totakeanexamplescenario:Apatientconsultsadoctor,whotakesabloodsampleandarrangestoseethepatientagainintwoweeks.a)A"good"practicedoctorseesandreviewsthebloodtestresultassoonasitcomesbackfromthelaboratoryandthenfilesitifnoactionisrequired.b)A"bad"practicedoctorseesandreviewsthebloodtestresultsonlywhenhereviewsthepatient’scaseonthepatient’snextvisit.Thiscaseisnotdefensibleifthepatienthasapreventableadverseeventandtakeslegalaction(source:MPSCasebookSummer1997).Thehealthcareinformationsystemputintothemedicalpracticeinelectronicformcouldbuild-ineitherpractice(a)orpractice(b).ThisreportdoesnotconsidertheclinicalqualityassurancemechanismfortheITsystem.
